This is a statement in which you are informed of the risks and hazards occurring whilst traveling to and participating in snorkeling or scuba dives either as a certified diver or as a student under the control and supervision of a certified instructor. This statement covers recreational scuba dive/snorkeling trips and scuba dive/snorkeling trips carried out as part of a class. This statement also sets out the circumstances in which you participate in the scuba diving/snorkeling trip at your own risk.
Your signature in the statement is required as proof that you have received and read the statement. It is important that you read the contents of the statement before signing it. If you do not understand anything contained in this statement than please discuss it with your instructor / dive professional. if you are a minor, this form must also be signed by a parent.
Warning
Skin and scuba diving have inherent risks which may result in serious injury or death. Diving with compressed air involves certain inherent risks, decompression sickness, embolism or other hyperbaric injury can occur that require treatment in a recompression chamber. Open water scuba diving/snorkeling trips may be conducted at a site that is remote, either by time or distance from such a recompression chamber.
In addition, during boat travel to and from dive/snorkeling sites, You should follow all safety instructions from the captain / crew members and take care while getting on or off the boat and while on board to avoid slipping, falling or drowning.
EXCLUSION OF LIABILITY
I understand and agree that neither (divemaster / instructor ) nor the crew of the vessel, accept any responsibility for any death, injury or other loss suffered of caused by me resulting from my own conduct or any matter or condition under my control which amounts to my own contributory negligence during or as a consequence of my participation in this scuba diving/snorkeling trip.
In this absence of any negligence or breach of duty by the divemaster / instructor / crew member or owner of the vessel, my participation in the scuba diving/snorkeling trip is entirely on my own risk.

Your signature all right full information requested on this form are necessary in order to participate in diving and related activities with ADM Venture Co.Ltd. / Scuba Quest Phuket Dive Center. Your health must be in good condition, any know health problems, and some of any medication or intoxicating beverages must be informed to your divemaster or instructor prior commencing any diving/snorkeling activity. Your divemaster or instructor will give you a prior diving/snorkeling briefing on all security matters in and out of the water.
You will also be informed about the technical parts of your equipment. Wrong handling of your equipment can be dangerous and can lead to serious injuries. I acknowledge above information and agree that the ADM Venture Co.Ltd./ Scuba Quest Phuket Dive Center and their employees are not responsible for any incidents happen in the duration of the dive/snorkeling education and/or during all water activities. By signing this agreement I agree to be fully liable for any incidents, injuries or death resulting from any foreseen or unforeseen circumstances. I also agree to be liable for any lost or damages to rental equipment. Tips and events, which I did book, but I don’t come on the departure day, I have to pay 100%.
I further state I am of lawful age and legally competent to sign this liability release or that I have acquired the written consent of my parents or guardian.
By signing and submitting this form, I acknowledge that I fully assume all associated risks. I waive any claims against the issuer or the recipient of this form arising from the use or processing of the information contained herein.

I hereby agree that I read and understood the exclusion of liability statement with Scuba Quest Dive Center Kamala (ADM Venture). I have also read the medical declaration and hereby state that I have not and do not suffer from any of the stated issues in the medical declaration.
